I've been looking for that for a while, but I never see it around.
Also, there was a reprint of "The Manual" about 2 or 3 years ago that he
wrote a new afterword for. It's excellent, and talks about his thoughts on
the book, ten years later.
(I don't have it around at the moment, but I could find publishing info if
anyone's interested)
